POSITION TITLE:    RN Care Coordinator - Cardiac
LOCATION:              New York City, NY, US 
COMPENSATION:    Base Salary - Based on experience
Position Overview:

 Position Requirements:
*    Under the supervision of the site manager, Department of Care Coordination, the care coordinator is responsible for coordinating and integrating high quality, cost effective care delivered to patients across an episode of illness or throughout the entire continuum of health delivery.
*Boldly advance patient care with our team-oriented, compassionate nurses.
 
Qualifications:  
      ·        RN with Master degree preferred. 3-5 years of health care experience in an acute care setting required.
·       Two years case management in an inpatient, HMO, Insurance or other outpatient setting preferred. One year of management experience preferred.
·       Current NYS licensure.
·        Basic computer skills.  Knowledge of Microsoft Work, Access, and Excel preferred. Strong organizational and leadership skills.  Excellent interpersonal skills.  Role model and change agent behavior.  Ability to problem solves and prioritizes, ability to function as core member of an interdisciplinary team.  Strong understanding of the business of health care.
